WORKERS' COMPENSATION INFORMATION

When an injury occurs at a Donna ISD facility that requires or may require medical attention: 

• Notify the Principal and/or Supervisor in charge. 
• Allow the school nurse to assess the situation when possible and complete the Nurse's Report of Accident Form. 
• Notify Safety & Risk Management department to provide the Employee's Injury Report and to receive instructions

Note: Donna ISD cannot authorize treatment without the Employee's Injury Report.
